Patient Access alows you to book, cancel and amend your appointments and request a prescription from the GP via this website.

Please ask for your unique log in details at reception, you MUST show a form of ID for your log in details or you will not be able to have them. You then click the above link to register.

 DETAILED CODED DATA - EXAMPLE:

From Wednesday 31st March 2016 you will have limited access to the following medical records online through ‘Patient Access’:

 1.) Medication history

 2.) Allergies/adverse reactions

 3.) Problems/diagnoses

 4.) Appointments

 5.) Demographics

 6.) Immunisations

 7.) Procedure codes (medical or surgical) and codes in consultation (signs, symptoms)

 8.) Results (numerical values and normal range)

 9.) Codes showing referral made or letters received (no attachments)

 10.) Medication (dose, quantity and last issued date)

 11.) Values (BP, PEFR)

 12.) Other Codes (ethnicity, QOF)

Please note that the above list is an example of catogories of information that patients may see. This may be presented differently by each GP surgery. 

If you would like to view your electronic medical record online please speak to one of the our receptionists to go through the following steps:

1.) We will require photographic ID

2.) You will need to complete a registration form

3.) Your request will take ONE WEEK to process

4.) One of our receptionists will contact you to collect your user ID and password.
