Home Blood Pressure Monitoring
If your blood pressure recordings are above target at your birthday clinic appointment, you may be asked to take some home blood pressure readings and return these to the Surgery.
If you wish to purchase a home blood pressure monitor, guidance on home monitoring and a list of approved home monitors can be found below on the British and Irish Hypertension Society website

If you are unable to complete the form online or you cannot scan or photograph a completed version to email, the same information can be typed into an email and sent.
Alternatively, if you are unable to email the results, please post these to the Surgery or bring the results to your birthday clinic appointment.
If your symptoms are deteriorating or you have any concerns, please make an appointment with the appropriate nurse or a doctor as well.
