  Are you a Carer?

A Carer is someone who provides unpaid care and support to a family member or friend who has a disability, mental or physical illness, substance misuse issue, or who needs extra help as they grow older.

Carers UK research in 2022 estimates the number of unpaid carers could be as high as 10.6 million (Carers UK, Carers Week 2022 research report).

We know that being a carer can be rewarding for many or hugely stressful, particularly if you don’t recognise yourself as one, and so are not getting all, or any, of the help and support you need. Many people care for family, friends or neighbours without realising that there is a range of help and support available to them.

If you recognise yourself as a carer, then please let us know so that we can update our records and help you to get the support and advice that you deserve.

Please make sure we have all your details on file by completing a Carer’s Identification form which can be downloaded or is available from our reception.

Carer Champion

Our practice has a dedicated Carer Champion, Maddy.

Maddy is the main contact person for our carers, staff members, local carer organisations and support services. She coordinates the identification and support of carers within St Peters Hill Surgery, acting as a voice for our carers.

Maddy maintains our carers boards around the surgery with useful information on a range of topics.

The Lincolnshire Carers Service is delivered in partnership by Lincolnshire County Council’s Care and Wellbeing Hub and Carers First. The Hub is the first point of contact for all new referrals and general enquiries regarding support for unpaid carers.

The Lincolnshire Carers Service is commissioned by Lincolnshire County Council. The service is delivered collaboratively between the Care and Wellbeing Hub and Carers First.

Whatever your circumstance, we offer services to support you at every stage of your caring journey to help you find the balance in your life you wish to achieve.

One to one support and programmes Our dedicated team is here to understand your specific needs and support you throughout your caring journey. Whoever you are looking after, we will equip you with the skills to help you thrive. We will talk through your situation, then tailor a programme of support, according to what you need in one or more of the following areas:

Health, Caring role, Managing at home, Wellbeing, Finances, Work

We have specialist advisors and programmes of support around employment, benefits and money matters. Our Health Team can support you with wider health matters, including support when someone goes into and leaves the hospital.

Carer Wellbeing Groups and Activities

A great way we can help is by connecting you with other carers. Our peer support and activity groups provide the chance to connect with people who understand.

There is something for everyone and our groups reflect the carers we support and the people they look after. From dance classes, book clubs and wellbeing groups to workshops for people in different caring circumstances.

Lincolnshire County Council Young Carers Service

Young carers can be a member of any family, from any background, and in any situation. The Lincolnshire Young Carers service helps and supports children and young people up to the age of 18 who are helping to care for someone at home.

How Are You (H.A.Y.) Lincolnshire is a family of websites that bring together everything in the local community that boosts wellbeing.

Lincolnshire County Council has teamed up with Carers UK to offer carers in our area a comprehensive solution that brings together Carers UK's digital products and Employers for Carers specialised practical online resources and simplified access to the jointly app and more through one website EFC Digital.

Visit the EFC Digital website and create a new account by using your free access code #EFC1947 for many new digital online resources.

The EFC Digital website includes e-Learning modules, factsheets, interactive guides, personalised support, and information from us split into the following areas:

Access to our care co-ordination app Jointly: Jointly is an app (also available on desktop) designed to make caring easier. It combines group messaging with other features such as to-do and medication lists, calendar and much more. You can use it with others or on your own.
