Confidentiality and medical records

If you would like to speak to a member of our team in privacy, please ask at Reception and we will gladly take you to a private room so you can discuss your needs in private without the risk of being overheard.

PLEASE NOTE THAT WHEN COLLECTING ANYTHING ON BEHALF OF ANOTHER PERSON, YOU MUST ALWAYS BRING IN YOUR THIRD PARTY AUTHORISATION FORM AND A FORM OF PHOTOGRAPHIC I.D.  WE ARE NOT BEING OBSTRUCTIVE WHEN ASKING FOR THIS, IT IS TO PROTECT OUR PATIENTS CONFIDENTIALITY.  WE DO HOPE YOU UNDERSTAND.

When ordering repeat medication via email, please be advised this request must go in with the GP for checking and may also go down to your designated pharmacy.

Parental responsibility.  The GP surgery can not act as mediator between parents.  Our responsibility is for the health and safety of your children and we can not be drawn in to any disagreements between both parties. Please see link below for guidance:

The practice complies with the new General Data Protection Regulations and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department. This requires your consent.
  • When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

GDPR

This is the new General Data Protection Regulations that comes into effect on Friday 25th May 2018.  It replaces the previous Data Protection Act and gives you as the Data Subject far more control over the use and retention of your data. 

You will be asked for your consent before information is shared with third party health professionals, ie District Nursing or Podiatry.

If you would like anyone other than yourself to collect your prescriptions, sick notes, test results, letters etc you will need to complete a consent form, again uploaded, and return it to the Practice.  We will not hand over any patient identifying information to a third party unless we have this consent.

SUBJECT ACCESS REQUEST -  Article 15 Right of Access (SAR)

If you wish to have a copy of all your personal information held by the Practice, please put your request in writing outlining what it is you require.  We will have this information ready for you in 28 days from the receipt of the request.  There will be no charge for this information.  You will be required to collect the information from the Practice with photographic i.d

Complaints

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can sometimes go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

To pursue a complaint please contact the practice manager who will deal with your concerns appropriately.

You are also entitled to direct any complaints regarding Ramsey Group Practice to Family Practitioner Services, please call 642613 for further details.
