Access to Medical Records

In line with current legislation, you have a right to access your medical records.  For further details please contact either the Secretaries or the Practice Manager. 

Your Information, Your Rights - What You Need to Know is a leaflet that explains why NHS Wales collects information about you and how this may be used.  It also tells you about your right to see your health records and how to gain access to them.

A copy of the Freedom of Information is available for all patients.  Please either apply for a copy through the 'Contact Us' tab on our email site or alternatively ask at the Receptionist, alternatively, a link is attached for you to download the document.

Why Does the NHS collect Information about you?

Your doctor and the team of health professionals caring for you keep records about your health and any treatment or care you receive from the NHS.  This information will either be written down (manual records), or held on computer (electronic records).  These records are then used to guide and manage the care you receive.  You may also be receiving care from organisations outside the NHS (like social services). If so, we may need to share some information about you so that everyone involved in your care can work together for your benefit.  Whenever this is necessary, your information will be handled in the strictest confidence and will be subject to the principles of confidentiality.  For further information follow the link below.

Complaints or Suggestions

Should you be dissatisfied with any aspect of the service you have received at this practice then please put this in writing for the attention of the Practice Manager.  This can be in written format using the below proforma and posted/handed to the receptionist or emailed via this websites feedback function. Please note that we are unable to guarantee the security of information sent via the website feedback function.We operate a complaints procedure as .  We aim to provide a high quality service to all our patients, and our Practice Manager would be interested in hearing your suggestions both positive and negative please feel free to email any comments to enquiries.w97028@wales.nhs.uk .

Violence Policy

The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
