IMPORTANT NOTICE - COVID VACCINATION

 Sandy Lane Surgery has opted into the scheme to administer the COVID vaccine.

 Leyland Primary Care Network – Sandy Lane Surgery, The Village Surgeries Croston & Eccleston, Worden Medical Centre, Moss Side Medical Centre and Central Park Surgery will be joining together to ensure this vaccine is rolled out. The Network are currently administering second vaccinations from Jubilee House.

The Leyland Collaborative

Practices in Chorley & South Ribble are working together in groups to offer weekend, bank holiday & weekday evening appointments for their patients. These additional appointments may be held at another surgery within the Leyland group rather than at Sandy Lane. Please be aware, regardless of the time or location of the appointment, you must ring Sandy Lane in normal opening hours to book or cancel an appointment. The group consists of the following practices

Patient Online Services

We now have nearly 4000 patients who are using our on-line service Patient Access to book appointments, order prescriptions, There is also an option to view your medical record including test results and hospital letters. For more information CLICK HERE

Do you know who your allocated GP is?

As part of our contract we must now inform all patients which GP they are registered with at the surgery. This is simply an administrative task and you are still able to continue seeing the GP of your choice. If you do not know who your allocated GP is, ask someone next time you come to the surgery  - thank you

Are available for minor queries or reviews if you do not need to be physically examined.

Use your Pharmacy for minor ailments

Remember your local pharmacist is trained to treat minor ailments such as coughs and colds, blood pressure checks, constipation, rashes / skin problems and headaches etc. Please ask at your local pharmacy.

NHS Health Checks

We offer NHS Health Checks free of charge to all patients age 40yrs to 74yrs, who are not being monitored in any of our clinics such as the CDM or Diabetic clinics. These NHS Health Checks are done every 5yrs. If you think you qualify for a health check or have not had one in the last 5yrs please contact reception.

Coronavirus remains an ongoing risk despite the Government easing of restrictions and will continue to be a real risk to us all for the foreseeable future

We are extremely grateful to all our patients who have made considerable efforts to manage their medical problems by themselves through this pandemic and request that everyone continues to be sensible and follow advice regarding appropriate safety measures and accessing support and advice from a variety of sources (pharmacist, optician, dentist, NHS 111) prior to contacting the surgery where this is appropriate.

We need to continue to do everything we can to protect the vulnerable members of the community and protect your surgery team so that we can continue to offer you an effective service.

Due to the ongoing coronavirus pandemic, your appointments at the surgery continue to be different for the moment. Your GP practice is open and appointments are being delivered face-to-face, online and over the telephone.

Face-to-face appointments, when an intervention is essential for your care, continue to be available to all patients.  Appointments with the nursing/HCA team for things such as blood tests, injections, including contraceptive injections and childhood immunisations, baby checks and cervical smears can be booked online or with the reception team. For the foreseeable future the vast majority of appointments with the GP/ANPs and long term condition reviews with the nurses, will be via telephone or video.

For appointments with the GP/ANP team you will initially be offered a telephone or video appointment to discuss your conditions and they will assess if a face to face assessment is essential in order to make a key decision on the next appropriate step. 

The safety of our patients remains a key priority, so we will continue to do what is necessary to achieve the right balance between keeping you away from the surgery and asking you to attend. Limiting face-to-face contact whenever possible is an important part of reducing the numbers attending the surgery and thereby reducing risk of virus transmission and keeping the surgery as safe as possible for those who need to attend.

Our opening hours are Monday – Friday 8am – 6pm. 

Please do not come to the surgery unless you have an appointment. If you are asked to come into the surgery for a face-to-face appointment, please remember to wear a face covering. Measures are in place to keep you safe from infection during your visit to the surgery.
