Have you missed classes due to illness?

In summary, if you have a period of absence of less than 6 weekdays, you can provide a self-certification.
If you have been absent for between 6 to 11 days and have seen a doctor or our nurse practitioner with regard to your illness, you can request a medical certificate (see below). If you have not seen a doctor or our nurse, you can provide a self-certification.
If you have missed classes on more than 11 days, you must submit a medical certificate. To obtain this you must have been seen by a doctor or nurse practitioner at the time of your illness.
If you have missed an examination or failed to submit an assessment by the required deadline, you may request a Senate letter (see below). Once again, you require to have been seen by one of the doctors at the time of your illness.
We may be able to write a Sick Note which provides dates of when you were ill. You need to have been seen by a doctor or our nurse Practitioner at the time of your illness. Charge £5
We may be able to provide Senate Letters which gives a bit more detail and requires to be typed on our headed paper. You would collect this from us after 7 days, although at our busiest times there may be a delay. You need to have been seen by a doctor or our nurse Practitioner, at the time of your illness. Charge £15.
Travelling abroad?

Require a note of your vaccinations?
Please come into the practice to a complete a request form. A Vaccination History form will then be completed by one of our nurses as soon as possible. There is a charge of £15.

It is very important that you keep us up to date with your address and telephone number, so that if we need to contact you, we have the correct details.
