Moving House? Important Information
From 1st April 2013, if you move house and your new address is outside of our ‘outer boundary’ you will be asked to register with a GP closer to your new address. Areas that are outside of our outer boundary include: parts of Stretton, parts of Horninglow, parts of Shobnall, Outwoods, Rolleston on Dove, Anslow, Tatenhill and Derbyshire. Please see the map on display in the waiting room for further information.
In order to be fair to all patients we are unable to make exceptions to this policy.
