Madeira Medical has a legal duty to explain how we use any personal information we collect about you as a registered patient at this practice. Staff at this practice maintain records about your health and the treatment you receive in electronic and paper format.

Register for Online Services

We are introducing a new service called Patient Access Online Services. This will let you interact with our practice using the Internet at your convenience, even when we are closed.

You will need to register with the practice to access this service by completing the form below. Once registered you will need to collect a letter from your reception (allow 2 working days) containing information that will enable you to create your username and password.

Note: When you collect your letter you must bring in one of the following to verify your identity:

Passport
Driving Licence
