Statement of Intent for Patient Online Services

This practice currently offers the facility for patients:

  • to order online their repeat prescriptions for drugs, medicines or appliances. 
  • to view online medications, allergies and adverse reactions

These services are available, on request, to any patients registered with this practice aged 18 and over. 

PLEASE NOTE: To register for online services patients will have to complete a form. This form can be picked up from Reception. As this service has access to confidential information such as medications, allergies etc, we do require one form of Photo ID (passport, photo driving licence). 

Please speak to a member of the Reception for further details. 

PROXY ACCESS

Up until a child's 11th birthday, the parents of a child will usually control access to their child's record and online services. Proxy Access will be switched off automatically by EMIS when the child reaches the age of 11. However, parents may continue to be allowed proxy access, after careful discussion with the GP, if it is felt to be in child's best interests. 

A practice may want to contact parents who have proxy access for a child approaching their 11th birthday, to remind them that their access is coming to an end and to invite them and their child to come to the practice for a discussion. 

Where a parent or carer is given proxy access for a young person after their 11th birthday, the patient's competence to make an indepedent and informed decision about proxy access should be re-assessed regularly. 

Once a young person turns 16 they are assumed to have capacity unless there is an indication to the contrary.
