You can now update some basic information in your clinical record - just click on Update Your Clinical Record link at the bottom of the home page and an online form will open which you can use to send us details about your height, weight, waist measurement, smoking status, if you are a carer or if you have any allergies.
You can also inform us of your ethnicity.
This information will then be added to your computer record at the surgery by a member of staff.
