Named GP
Every patient registered at the practice has a named accountable GP.
Although this is a new initiative nationally, we have already been doing this for many years, recording a patient’s ‘usual doctor’.
This means that there is one doctor in the practice who is responsible for coordinating your care
At Horton Park Medical Practice, the GPs work closely together; this does not
prevent you from seeing any other doctor at the practice.
Please be aware that due to holidays and other commitments, your named (usual) GP may not always be available.
If you request an emergency appointment, this may be with any of the of the GPs
You can change your accountable (usual) GP to another GP of your choice if you so wish.
If you would like to know who is your accountable (usual) GP please ask at your next appointment.
