SOCIAL PRESCRIBING 

A Social prescriber Link Worker is at your GP Surgery

I will support, signpost and help with information for you with any none medical needs such as:-

                                     Loneliness/Anxiety

                                         Weight Management

                                          Welfare Rights and Benefits

                                             Substance misuse 

                                               Food Poverty (Urgent)

This is a self-referral service and or via the GP Surgery. It will be an initial telephone call with an option of face to face to follow. The aim is to contact the patient within 2 weeks of the initial referral.
