Social Prescribing

Your doctor isn’t the only person who can help you feel better. You can improve your own health and wellbeing through social prescribing.  

You don’t need a formal referral from your GP either; you can contact the service direct where you will be given the opportunity to meet a community connector who can spend time with you exploring what activities and local support could improve your health and wellbeing. You can then consider the benefits of participating in them and we will assist you in planning your next steps.

Supporting a range of issue such as;


           social isolation

           emotional wellbeing

            healthy life choices

            getting out and about

            life changing events such as a birth, retirement, bereavement

            long-term health conditions

            loss of confidence/purpose

            poor health linked to housing conditions

            accessing work, training and volunteering
