IMPORTANT NOTICE - COVID VACCINATION

 Sandy Lane Surgery has opted into the scheme to administer the COVID vaccine.

 Leyland Primary Care Network – Sandy Lane Surgery, The Village Surgeries Croston & Eccleston, Worden Medical Centre, Moss Side Medical Centre and Central Park Surgery will be joining together to ensure this vaccine is rolled out. The Network are currently administering second vaccinations from Jubilee House.

The Leyland Collaborative

Practices in Chorley & South Ribble are working together in groups to offer weekend, bank holiday & weekday evening appointments for their patients. These additional appointments may be held at another surgery within the Leyland group rather than at Sandy Lane. Please be aware, regardless of the time or location of the appointment, you must ring Sandy Lane in normal opening hours to book or cancel an appointment. The group consists of the following practices

Patient Online Services

We now have nearly 4000 patients who are using our on-line service Patient Access to book appointments, order prescriptions, There is also an option to view your medical record including test results and hospital letters. For more information CLICK HERE

Do you know who your allocated GP is?

As part of our contract we must now inform all patients which GP they are registered with at the surgery. This is simply an administrative task and you are still able to continue seeing the GP of your choice. If you do not know who your allocated GP is, ask someone next time you come to the surgery  - thank you

Are available for minor queries or reviews if you do not need to be physically examined.

Use your Pharmacy for minor ailments

Remember your local pharmacist is trained to treat minor ailments such as coughs and colds, blood pressure checks, constipation, rashes / skin problems and headaches etc. Please ask at your local pharmacy.

NHS Health Checks

We offer NHS Health Checks free of charge to all patients age 40yrs to 74yrs, who are not being monitored in any of our clinics such as the CDM or Diabetic clinics. These NHS Health Checks are done every 5yrs. If you think you qualify for a health check or have not had one in the last 5yrs please contact reception.

OOverview of SCR ~ (Summary Care Record)

Summary Care Records (SCRs) are an electronic record of key information from a patient’s GP practice. Over 96% of people registered with a GP practice in England (55 million people) now have a Summary Care Record. The benefits include improvements in patient safety and experience and in the efficiency and effectiveness of patient care.

 SCRs are viewed in a wide variety of care settings including:

  • Acute Trusts (both unscheduled and scheduled care)
  • GP Practices
  • GP Out of Hours services
  • Walk in Centres
  • Urgent Treatment Centres
  • Minor Injury Units
  • Mental Health
  • Community Pharmacy
  • Custody Suites
  • Prisons
  • Hospices

Community Care

SCRs contain the ‘core’ dataset of medication, allergies and adverse reactions.

SCR’s are optional but the national opt out rate remains low at 1.4% and a patient can change their mind at any time simply by contacting their GP practice.

Patients can also choose whether they wish to share the core data in the SCR only or whether they wish to have an enriched SCR i.e. include important additional information from their GP record.

Increased patient safety by providing timely access to information such as significant diagnoses, significant medical history past and present, patient preferences for resuscitation, power of attorney, communication preferences, end of life care information
  • The patient can choose to add more information if they feel it is necessary

  • Increased efficiency in treating a patient anywhere in the world at any time of day
  • The Enriched information is quickly available to healthcare staff who already routinely access SCR’s
  • Results of Tests, x-rays or procedures done at one hospital can be accessed by the clinicians at other healthcare facilities to improve your care
