Named GP

Named GP’s. All patients are automatically registered with Severn Fields Medical Practice (previously Mount Pleasant Medical Practice & Haughmond View Medical Practice) . As part of a government initiative patients have also now been allocated to a named accountable GP. It is important to note that patients are still free to see whichever GP they choose however any incoming post of results will be reviewed by your named GP and will have overall responsibility for your care. If you would like to know who your allocated usual GP is then please speak to any member of staff when you next attend the practice or alternatively ask at receptionist.

Q What Does Accountable Mean?


A Your named accountable GP will oversee the co-ordination of your care amongst the other health professionals.


Q Does this mean that my named accountable GP will take 24hr responsibility for my care?


A No, your named GP will purely oversee the care that you receive.


Q Does this mean that I have to see my named GP every time I book an appointment?


A No, a patient can express who they would like to see, however, this may not always be possible due to appointment availability with that GP.


Q Can patients still see any GP they wish?


A Yes, a patient can continue to see any GP they wish as they currently do.


Q Will I be informed in writing who my accountable named GP is?


A No, your practice does not have to inform you in writing. If you would like to know who your named GP, however if you would like to know the named of your accountable GP please speak to a member of the reception team who will be happy to .
