Are you on the Carers Register?

Mortimer Surgery is compiling a Carers Register to identify and support patients who are also unpaid carers.

The surgery aims to:

  • Ensure carers have an annual health check, are offered annual flu vaccinations and are screened for problems such as depression and anxiety
  • Check that appropriate benefits and allowances have been offered and that emergency plans have been discussed for times of crisis

Please ask at reception for a form to register as an unpaid carer

If you would like more information about support for unpaid carers, please contact the Social Prescriber at Mortimer Surgery
