Quality Improvement Activity

Wyre Forest Health Partnership Quality Improvement Activity:

We aim to instill a culture of continuous quality improvement in all levels of our organisation. This page includes some examples  of this work  for our sites.  We are sharing the documents relating to these audits to give examples of the breadth of our work  these audits have already ben share within the sites to help improve care.

Board Level:

The board regularly discuss performance and quality data for our organisation, an example of our board report is shown below, some elements of this report have been removed in case they identify individual patients. but 99% of the report is shown here for information.  This report is discussed and actions documented in the board papers. These actions are not visible on this report which is included on the website as an example of our work

We also regularly benchmark performance for al our enhanced services by site and use this to help the sites to improve.

We have a wealth of knowledge about our sites including data on performance and financial data, are currently going through a process with all our sites where we meet the site partners and site manager to discus performance and areas for improvement to mutually agree a site development plan for the next 12 months.
