Cardiovascular disease and diabetes – assessing your risk


 We wish to invite you to attend for screening of heart disease and stroke or diabetes in the future.  This service will involve two appointments. 


The first appointment will be with a healthcare assistant who will check your height and weight, blood pressure, take a fasting blood test for cholesterol and sugar levels (please fast for 3 hours before your appointment, you should take your usual medication with a sip of water).  We will ask you some questions about your family history of heart attacks, strokes or diabetes and your health and lifestyle. At this appointment you will book in for your follow up appointment.


After your first appointment you will be sent a letter with your test results and information to help your understand them. This will give you the chance to think about what you want talk about during your next appointment. Have a look through this information and write down your questions, concerns and comments.


You will then have a follow up appointment.  At this appointment you will be able to ask questions and talk about what is important to you. We will look at your results and discuss options that might work for you, helping you to decide on one or two priorities and what support you might need to help you look after your health.
