If you would like to be able to see your records, order prescriptions and make appointments on line please speak to reception and they will be able to give you access providing you can show us ID to verify your identity. This should be a birth certificate, driving licence, passport or marriage certificate. In addition one form of ID with your current address, e.g. your bank statement or council tax bill.
