Community & Care Co-ordination Scheme

In partnership with Severn Fields Medical Practice (and other practices in Shropshire) we are offering a new service to provide advice and support for patients who are carers.

Often people do not identify themselves as being carers however if you look after someone who is ill, frail, disabled or mentally ill we are here to help.

Our aim is to assist patients who are feeling isolated or lonely, or who are struggling to cope with life at home.  The Care Co-Ordinator can help to put in place support or find activities provided by the many voluntary and statutory services in your community.

Examples of the support available are as follows:-

Befriending service - a volunteer will call for a chat and a cup of tea.

Signposting to support services

Help with filing in forms

Transport to and from medical appointments - a volunteer will stay with you throughout your visit.

Help with collection of prescriptions and or pensions.

Help with the shopping

If you, or someone else feel that you might benefit from this service please visit your GP and ask to be referred to the Community & Care Co-ordinator

 

Whatever the problem, she will listen and do her best to help.
