Patient Registration

Registration with the practice

We welcome new patients, and registration is easy.  Please look under 'Practice Area' to see whether your address falls within our catchment area.  You simply need to come down in person to the practice during opening hours, ideally between 9.00am-5.30pm (please note we close between 1.00-2.00pm for lunch). Here you will be asked to fill in a brief registration form. You will need proof of identity and address, (passport/driving licence and household bills, for instance). You must also bring your NHS number (which you can obtain from your previous GP if you do not know it) and the details of your previous GP (if you have one) with you.  Please note that if you are registering more than one adult then each adult has to come into the practice in person. You cannot bring in completed forms and ID on behalf of another adult.

If you are registering a child or new-born baby, please would you bring in the child's Red Book.  If no Red Book is available, please bring in the child's birth certificate or passport.  In addition, we ask you to bring in one of the following: child benefit form, NHS card or, if you do not have either of these, the child's passport.
