Are you ready to achieve goals to improve your health and wellbeing?

What do we do?

It is sometimes hard to make a change by yourself. Health Trainers can help you set health goals and support you to achieve them. You may want to control your weight, get fitter, have a healthier diet. Or, you may want to reduce your stress levels or just feel better about yourself.

How does it work?
We offer a free confidential service providing one to one support. A session can last up to an hour. This will give you time to discuss any issues that you feel are affecting your health and the chance to look at positive solutions.
We can provide up to 6 sessions if you need them. You may be introduced to a new organisation or activities, and if you wish it may be possible for us to accompany you on the first visit.

We have male and female health trainers who can also speak a range of community languages working in various locations across the district.

We offer:

  • Free and confidential 1:1 support
  • To set and achieve SMART (Specific Measurable Achievable Realistic Timely) goals
  • Support that is delivered through motivational interviewing / coaching / solution focussed questioning
  • To help identify barriers and enablers to achieving the goal
  • To  provide encouragement, motivation and praise
  • To  understand and help if there are setbacks with achieving a goal
  • To provide up to 1 hour per appointment and up to 6 appointments
  • Signposting / social prescribing to voluntary community sector support and other services / groups / activities
  • To refer on to other NHS services e.g. District Nurses, Primary Care Mental Health Team
  • To accompany patients  to groups / activities
  • Home visits (where necessary)
  • A follow-up 3 months after the date of last appointment
  • Telephone support
  • Group support
  • 12 week Healthy Weight Programmes

About the Service:

  • We are a client-led service and do not offer advice
  • We work with clients with or without long term conditions
  • We work with people who experience health inequalities
  • Health Trainers:
    • Are non-clinical lay staff who use a peer support approach – “support from next door not advice from on high”
    • Help patients achieve “small changes to make big differences”
    • Are representative of the population and speak various languages
    • Have had similar experiences as those of our clients
    • Are approachable, non-judgemental and competent
    • Are self-employed, freelance, sessional workers based in various local and accessible organisations across the district e.g. Community Centres, Children’s Centres, GP practices, Health Centres, Town Halls...
