From Thursday 28th March 2019 all prescriptions for schedule 2&3 controlled drugs will be sent electronically to your nominated pharmacy. If you do not have a nominated pharmacy there will be no change in the way you collect your prescription.

Practices in Chorley & South Ribble are working together in groups to offer weekend, bank holiday & weekday evening appointments for their patients. These additional appointments may be held at another surgery within the Leyland group rather than at Sandy Lane. Please be aware, regardless of the time or location of the appointment, you must ring Sandy Lane in normal opening hours to book or cancel an appointment. Patient Online Services

We now have nearly 4000 patients who are using our on-line service Patient Access to book appointments, order prescriptions, There is also an option to view your medical record including test results and hospital letters. For more information CLICK HERE

Do you know who your allocated GP is?

As part of our contract we must now inform all patients which GP they are registered with at the surgery. This is simply an administrative task and you are still able to continue seeing the GP of your choice. If you do not know who your allocated GP is, ask someone next time you come to the surgery  - thank you

Are available for minor queries or reviews if you do not need to be physically examined.

Use your Pharmacy for minor ailments

Remember your local pharmacist is trained to treat minor ailments such as coughs and colds, blood pressure checks, constipation, rashes / skin problems and headaches etc. Please ask at your local pharmacy.

NHS Health Checks

We offer NHS Health Checks free of charge to all patients age 40yrs to 74yrs, who are not being monitored in any of our clinics such as the CDM or Diabetic clinics. These NHS Health Checks are done every 5yrs. If you think you qualify for a health check or have not had one in the last 5yrs please contact reception.

Coronavirus (COVID-19) and requests for sick notes (Med3)

Dear Patient/s 

We are receiving a lot of enquiries about sick notes from patients.  This is meaning that our telephone lines are blocked for patients who are unwell.  We have produced the following guidance that we hope will answer your questions.

You may want to obtain information about the following categories:

Personally affected – isolating for seven days

For any period of illness less than seven days, you are able to complete a self-certification certificate and provide this to your employer. A SC2 form can be downloaded from the internet. You do not need to contact your GP.

Personally affected - remaining unwell for over seven days

If patients remain unwell and unfit to work over seven days, advice is to visit www.111.nhs.uk and complete an online self-assessment. The patient may be asked to call 111 for further advice on the next steps regarding their illness/ fitness to work. You do not need to contact their GP.

Isolating as per government advice

In light of the current Coronavirus situation, it is inevitable that a number of patients may be required to isolate themselves at home, for example due to the nature of recent travel, or because other members of the household are showing symptoms even in the absence of any symptoms of an illness. In these cases, your GP will not be able to issue a sick note as the time off work is not due to an illness, and isn’t affecting your ability to work, but rather is a precaution to avoid transmission of any illness that is yet to produce symptoms.

We therefore expect your employer to be sympathetic if you are required to self-isolate for 14 days and to understand that the reason for this is to protect the welfare of your work colleagues and the wider community.

In these circumstances you can obtain a sick note for your employer if needed from the NHS 111 online service:

 At risk group – following government advice

Employers are responsible for putting in place arrangements for home/remote working where this is possible. Where it is not, the employee may self-certify and return to work following the relevant absence which their employer may authorise as per government advice. Where they do become unwell during or after this time, point 1 and 2 applies. You do not need to contact your GP.

Whilst we appreciate this may cause some difficulties between you and your employer, equally GPs have a responsibility to prioritise the assessment and management of the healthcare needs of patients who are acutely unwell, rather than spending time dealing with requests for sick notes, particularly when they aren’t in a position to issue one.

Further Update re:  Practice Coronavirus arrangements with effect from Wednesday 25th March 2020

 With the aim of protecting ourselves and our patients 

In response to the current coronavirus pandemic, GP Practices across the UK have changed how they deliver care to their patients.  We would ask for your support as we implement the following changes.

Please do not attend the practice unless you have been asked to do so by a clinician.  If you attend the practice and you have not been asked to do so you will be asked to go home and communicate with us by telephone.

If you are asked to attend the practice you will be asked to provide us with a mobile telephone number and then wait in your car or outside the building until the clinician telephones you and asks you to enter the building.

All appointments have been/will be converted to telephone appointments.  Patients will be contacted by text message to change or cancel appointments.

 A limited number of appointments will still be able to be held face to face, as long as you have no high temperature or respiratory symptoms.  These include:

  • Prostap/Zoladex 
  • INR
  • WBC/Childhood immunisations
  • Midwife

 These appointments will be contacted the day before to be triaged.  Should you, or anyone who is bringing you to that appointment develop any new respiratory symptoms or fever you must cancel the appointment.

Long term condition reviews will be either conducted by telephone or cancelled and deferred until later in the year.

Routine blood tests will be cancelled.  Clinician initiated urgent blood tests will still be seen.

Please delay ringing to book routine smear tests at the moment.  Please ring us in 3 weeks for further guidance.

Please do not bring your prescription requests to the surgery unless absolutely necessary.  Please see the separate prescription guidance.  If you are collecting a paper prescription that has already been ordered please knock on the door and speak to the receptionist.

Do you have a high temperature or a new continuous cough?  If so you must self-isolate at home for 7 days, to slow the spread of the virus.  If you are still unwell after 7 days, or you feel unable to cope with the symptoms at home, you must contact the 111 Coronavirus service.  Please use the online tool where possible or call 111.

Disabled Access

The practice has made every effort to ensure easy access to the building and toilet facilities are available for the disabled. All patient facilities are located on the ground floor of the building and part of the reception desk has been lowered to facilitate wheelchair access.

There is a loop system for the hard of hearing. Please inform our reception staff if you wish to use this.

Breast feeding and baby changing

The practice is happy to accommodate breast feeding mothers and will allocate a private room on request. Baby changing facilities are available in both the male and the female toilets.

Pharmacy

Rowlands Pharmacy is attached to the surgery and is used by the whole community.

Parking

We have a limited number of car parking spaces at the surgery, with 4 disabled parking bays. This is accessed from Sandy Lane.

Public Transport

Several bus services run close the Surgery. The closest bus stop to the Surgery is at Tesco, Leyland . There is a short path along the side of Tesco which leads towards the Surgery. It is no more than 2 minutes walking distance.
