Welcome to Rise Park Surgery

Rise Park Surgery serves the whole of Rise Park as well as areas of Top Valley, Bestwood Park and Bestwood Village.

Our team includes seven GPs, two Nurse Prescribers, one nurse, one Nurse Associate and two health care assistants as well as our Business Manager, Patient Services Manager, administration and reception staff. We offer a full general practice service and run specialist clinics for children and pregnant women, diabetes and asthma sufferers and for patients needing minor surgery.

Appointments at Rise Park Surgery

If you need an appointment or you have a general query (e.g. fit note request, results, doctors letter, referral etc) then please click the link below and complete the form.  Please answer ALL questions as fully as possible including the times that you are unavailable to see us or be contacted. If you don't tell us when you are not available and we offer you an appointment that you later cannot make you will be asked to resubmit your request the following day.

Our preferred way to contact you to advise you as to what will happen next is by text message so please keep your phone to hand and look out for a message from us. For example we may offer you a telephone call within a 2 hour window or ask for you to come down to the surgery if the clinician feels they need to see you for a 'face to face' appointment. 

If the clinician is going to telephone you please be aware that they will try to contact you twice and if they are unsuccessful in speaking with you they will ask you to resubmit your request the following day. 

We hope that this will be a more convenient way for many of our patients to contact us and it will hopefully reduce the waiting times to reach a reception colleague via the telephone for those that prefer to use the phone.

Your request will be acknowledged within 48 hours (working days when we are open). 

If your condition worsens please telephone the surgery or call 111 or 999 in the case of a medical emergency.

Just to let you know …

A

The Surgery will be closed on from 1230h on Tuesday 23rd January 2024 for staff training.

We will re-open at 0800h on Wednesday 24th January 2024.

Any calls to the surgery will be automatically diverted to the NEMS Out of Hours Service or alternatively please call 111 if you need urgent medical attention.

How your data is used

Your health and care information is used to improve your individual care. It is also used to help us research new treatments, decide where to put GP clinics and plan for the number of doctors and nurses in your local hospital.
Wherever possible we try to use data that does not identify you, but sometimes it is necessary to use your confidential patient information.

 

What is confidential patient information?

Confidential patient information identifies you and says something about your health, care or treatment. You would expect this information to be kept private. Information that only identifies you, like your name and address, is not considered confidential patient information and still may be used: for example, to contact you if your GP practice is merging with another.

 

Who can use your confidential patient information for research and planning?

Is used by the NHS, local authorities, university and hospital researchers, medical colleges and pharmaceutical researching new treatments.

 

Making your data opt-out choice

You can choose to opt out of sharing your confidential patient information for research and planning. There may still be times when confidential patient information is used: for example, during an epidemic where there may be a risk to you or to other people’s health. You can also still consent to take part in specific research project.

 

Will choosing this opt-out affect your care and treatment?

No, your confidential patient information will still be used for your individual  care. Choosing to opt out will not affect your care or treatment. You will still be invited for screening Services, such as screenings for bowel cancer.

 

What should you do next?

You do not need to do anything if you are happy about how your confidential patient information is used.

If you do not want your confidential patient information to be used for research and planning, you can choose to opt out securely online or through a telephone service.

 

You can change your choice at any time.

Order your repeat prescription online!


Rise Park Surgery are pleased to announce that we are able to offer patients the ability to order repeat prescriptions via this website as well as viewing your medical record.

To register for the website please bring one form of photo ID (i.e. passport, driving license) and one utility bill showing your current address (no more than 6 months old) and reception will provide you with a user name and password.

We therefore ask that unless your problem is urgent and you need an appointment that morning please phone after 11:00am. Thank you.
