EASTER OPENING TIMES
We will still be here for you over Easter.
The practice will be open on both Good Friday (April 10) and Easter Monday (April 13).
Phone lines will be open from 8am until 6pm to ensure that any urgent needs can be responded to by an appropriate practice clinician.
Please do remember you should not attend the practice without an appointment, as you will not be seen.
If you need to contact us, please do so by ringing the normal practice telephone number.
For the rest of the weekend, the extended hours 'GP Care Wakefield' service is running as usual between 9am and 3pm. To access the service, patients should ring the usual practice number to be redirected to a trained clinician who can assess their issue and arrange a telephone consultation, or appointment locally, if needed.
As you will be able to appreciate, this is an unprecedented time where our staff are extremely busy. Your cooperation, understanding and patience are therefore greatly appreciated.

There is no need to go to your GP Practice if you require an isolation note for your employer.

If you have been told to stay at home because of coronavirus and you need a note for your employer.
This service is only for people who:
- have symptoms of coronavirus and have used the 111 online coronavirus service
- have been told by a healthcare professional they have symptoms of coronavirus
- live with someone who has symptoms of coronavirus
If you are not sure if you need to stay at home, get the latest NHS advice on coronavirus.
If you have to stay at home but feel well enough to work, ask your employer if you can work from home. If you can work from home, you will not need an isolation note.
You can also use this service for someone else.
If you, or a friend, family or neighbour does not have access to the internet, please telephone 111 and they can complete it for you.

URGENT MESSAGE DUE TO LATEST CORONAVIRUS (COVID-19) AFFECTING APPOINTMENTS AND SERVICES FROM MONDAY 16.03.2020
Due to the ongoing situation with coronavirus, we have been advised by NHS England and Public Health to make changes to some of our current services. This is to minimise risks associated with the virus. Therefore, we are asking you, our patient(s), to support us with this and for you to try and keep up to date with changes. This will help to protect you, your family and friends, people more at risk than others (identified by Public Health) and also our team at the practice. We all want to remain as well as possible and work through this extreme situation together.

We would like to inform you of changes within the appointment system / services and how we will, until further notice, help patients, to enable us to manage the increased pressure and workload, trying to keeping everyone as safe and healthy as possible including our workforce:-
Appointments and advice
As you will be aware, we have already suspended the booking in advance (pre-bookable) appointments and the ability to book appointments on line. This will continue until further notice
We are asking ALL patients not to attend the practice to request an appointment or seek advice.
Public Health has advised all Practices to minimise the number of patients attending the Practice for face to face appointments. This means that all patients are required to contact the practice by telephone to request an appointment and provide details to our reception staff of your need. Reception staff WILL continue to ask for a brief description of the problem to understand your needs and identify if you meet any of the corona virus symptoms.
A member of the clinical team will then phone you back to discuss your need, and where ever possible, this need will be met over the telephone. As well as speaking via phone, we are also looking at options of offering a video conference or skype conversation if appropriate. Should the clinician deem that you require a face to face appointment, you will be asked some further screening questions to determine whether there is a need for a coronavirus assessment. If this is the case you will be asked to self-isolate for 7 days. You will NOT require a sick at this stage and will complete the usual self-certification form to pass to your employer (if you are in employment). If there is no risk, you will be given a face to face appointment.
As part of Covid-19 guidance, it has been made a criminal offence for patients to obtain access to GP Practices by not providing truthful symptoms. Any patients who are found to be giving false symptoms and who do present with a cough and / or fever of 37.8 or higher will be asked to return home as per national covid-19 guidance.
Existing appointments already booked
If you already have a pre-booked appointment in the near future, with a GP or Nurse Prescriber, you will be contacted (if not already done so), and the appointment will be turned into a telephone slot or cancelled. You will be contacted by one of our clinicians by phone. The clinician will then assess whether your medical need can be managed over the phone or whether they need to see you face to face; if the clinician feels that a face to face assessment is required they will arrange an appointment with you.
Existing appointments with Nurses are currently being reviewed. You will be contacted before the day of your appointment, to check your health situation regarding the coronavirus current screening questions and this will determine the course of action. We are aiming to continue delivering long term condition reviews but by phone, video or skype where possible. If your appointment can be delayed, you may be asked to contact the practice at a later stage. If it is determined that you require a face to face appointment, the Nurse will arrange this with you
Until further notice we will not be offering the following services
Routine blood tests
Spirometry or nebuliser use
New baby 6 week check examinations and mums post-natal checks
Routine follow-up appointments
NHS Health Checks
Extended hours are currently under discussion and should you have a booked appointment already – you will be informed before the day of you appointment
At this moment, we are hoping to continue delivering some face to face appointments with our Nurses for
Urgent blood tests
Prostate cancer follow up / injections,
Childhood immunisations and
Whooping cough vaccinations for pregnant women.
But these appointments will need to be discussed and arranged via one of our clinicians – so you should telephone the practice if any of the above will be required, for the latest advice. When speaking with the clinician, he / she will ask the same further screening questions as for an appointment with a GP or Nurse Prescriber (when deeming if safe to bring individuals into the practice)
To order a repeat prescription, please either:
- Continue to request online if you are set up for online ordering of your repeat medication
- Whilst there is limited access into the surgery, if you are not signed up to our specific online services, you could email us direct at firstname.lastname@example.org
- By post
- During opening hours you could pop it through our letterbox which can be found at the side door of the building (patients car park side)
- During the COVID-19 concerns, the Practice will also allow telephone calls to order your repeat prescription. Please call after 10am should you use this option to order your repeat medication. This service will only remain available until normal services are resumed.
We will electronically send your prescription to your nominated pharmacy. If you do not have a nominated pharmacy, please contact the practice by telephone or email email@example.com and you can inform us where future emails should be electronically sent.
Access to the surgery:
After speaking with a clinician and told you can attend the surgery for a booked appointment, please be aware we need to manage who enters, therefore
- Access to the building will be restricted to the patients who have been invited for an appointment by a clinician
- Wherever appropriate, only the patient who has a face to face appointment will be allowed into the building
- For children, only one parent/guardian will be allowed to accompany the child These decisions have not been taken lightly by the Practice and are being made in line with all national guidance around Covid-19 with the safety of patients and staff being absolute paramount. Arrangements will continue to be reviewed on an ongoing basis. Please remain courteous to our staff at all times; any incidents of verbal or physical abuse will result in removal from the practice list. This is a very challenging time for us all and we would appreciate your co-operation during this period of uncertainty for all of us. Thank you.
CROFTON AND SHARLSTON MEDICAL PRACTICE - WELCOME TO OUR WEBSITE
We aim to provide a high standard of medical care in a friendly and professional manner. The practice area lies to the east of Wakefield and centres on the ex-mining villages of Crofton with Foulby, Nostell and Wragby and Sharlston with New Sharlston and Streethouse. It includes the Heath Common, Warmfield and has traditional links to Walton, and skirts Sandal and Agbrigg.
It is bounded to the west by the City of Wakefield, to the north by the River Calder and Normanton to the east by Featherstone, to the south by Havercroft and Haw Park. As well as the core villages it covers a large tract of farms and countryside and includes Nostell Priory Estate, Walton Hall and Anglers Country Park.

The majority of our staff have undergone training to become a dementia friendly practice to help make visiting the practice an easier experience for those patients suffering with dementia.
Our building(s) were the first in the area, to trial the suggested risk assessment tool kit based specifically for GP practices and were able to work with our council and public health colleagues in making the building risk assessment tool, fit for purpose.
