You can contact us about a medical, administrative or prescription issue by clicking on the button below Monday to Friday. Answer a few short questions and we will get back to you within two working days. PLEASE DO NOT USE THIS FORM FOR URGENT OR EMERGENCY REQUESTS. The submitted forms will only be read during office hours.

If you have a medical request you are likely to be booked a telephone appointment by the practice team.

Please be careful to let us know any times when you are NOT available on the form within the online system, the GP or nurse will try twice but will then have to move onto other patients and your appointment will be flagged as did not attend. Thank you for your help with this.

WE ASK THAT ALL USERS OF THE MEDICAL CENTRE COVER THEIR FACES WHEN THEY ENTER THE BUILDING. A SCARF IS OK IF YOU DO NOT HAVE A MASK.

It is really important to remember that all the 'normal' illnesses that can make us unwell still remain and that you should still contact the surgery if you or your child are feeling poorly with symptoms that don't suggest COVID 19. 

Due to the coronavirus outbreak how you contact us will be different at the moment. This is to limit face-to-face contact whenever possible and help stop the spread of coronavirus.

The medical centre is open and if you need to see your GP please call us on 01937 530082 or use the online consultation link on this page.

You can also call NHS 111. Please do not come to the surgery unless you have an appointment.

If you are waiting for a check-up please be assured we will be in touch.

Appointments are being delivered face-to-face, online and over the telephone. If you are asked to come to the surgery for a face-to-face appointment please wear a face covering. Measures are in place to keep you safe from infection during your visit to the surgery.
