PRESCRIPTION CHANGES

From 12th November 2018 pharmacies can no longer request prescriptions on your behalf. Medication can be requested ONLINE, IN SURGERY or VIA LETTER. The surgery can not accept requests more than 7 days before you run out of medication. Please allow 3 working days for the Doctor to process your request. 

ID WILL BE NEEDED TO COLLECT ANYTHING FROM THE SURGERY

From 25 May 2018 photo ID will be required to collect prescriptions, med 3 (sick notes) letters, referral details or anything else from the surgery. If you wish someone to collect on your behalf please complete a consent form prior to this and return it to the surgery. Click to print a THIRD PARTY CONSENT FORMS.

SMS MESSAGING

We may sometimes inform you about your test results / diagnosis via text message. By giving the surgery your mobile number consent to receive information via text message is implied. If you do not wish the surgery to contact you or via text please inform reception.

Please make sure you give your full name and date of birth as well as the medication and dose you require and a contact telephone number in case we need to confirm any details.

Emergency Treatment after RTA's.

If you are seen at the surgery for emergency treatment after a road traffic accident you will be required to pay a fee of £21.30 which can be claimed back under the drivers insurance, see the main page for Further Information 

YELLOW FEVER CENTRE

We are now a Yellow Fever Centre and can offer Yellow Fever vaccinations to anyone who requires this.  You do not have to be a patient here to receive a Yellow Fever vaccination. Please contact the surgery to arrange an appointment with the nurse. The cost is £59 - this is for the vaccination and the certificate, payable in cash only at reception

LATE FOR AN APPOINTMENT - if you are late for the start of your appointment the clinician may not be able to see you. This is to reduce other patients wait time by sticking to their appointment times

Do It Online

ONLINE APPOINTMENT BOOKING IS COMING BACK FROM 5TH JUNE 2023.

You will be able to book appointments, order prescriptions and view results using systmonline or NHS App.

Online services allow you to access a range of services via your Computer, Smart phone or Tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results.

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a mobile phone or electric bill. If you don’t have these, please talk to us and we will help you sign-up. In order to see part your record we will require a further piece of photo ID. We reserve the right to refuse online services if we feel the need.

To use this new system, you will require a username and password. If you do not have one, contact the Surgery or ask a receptionist for more information.

Important Information:

After collecting your form (or being told the details). Please allow 2-3hrs for the website to add your account (you may notice that if you try to enter you details before, that it will not recognise them).

Every clinicians appointments are released by rotation at 07:45.

There are certain appointments you will NOT be able to book online.
