Keep your contact details up to date

If you have registered for Patient Access, you can update your details online. If you have not yet registered for this, please send any change of address or contact details to us in writing or fill out one of our change of details slips at reception.

Access to your own medical records is available using patient access. You will need to register for this service even if you are already registered to make appointments and order prescriptions.

Download the Patient Access registration form which you will need to complete and return to reception along with TWO pieces of ID, one piece of photographic identification and one recent non photographic correspondence (e.g. bank statement etc, please note, we are unable to accept utility bills) to complete the registration process.

Patient Access Terms and Conditions

Before you begin to use Patient Access, we would appreciate it if you could read the following terms and conditions.

Request a sick note extension (Med3)

To request an extension to a Med3, please email bobicb-bw.umg@nhs.net with the reason for your request and the dates requested. Please allow 5 working days before chasing. Your request will be sent to a GP who will either issue the Med3 or if they need further information, reception will get in touch to advise of next steps. For new requests, please make an appointment with a GP by calling us on 0118 9874551 at 8am Monday to Friday. 

Repeat Prescriptions

Patient Access will give you a facility to send repeat medication requests to the surgery. We will still require 2 working days to process requests, and if you request medication not on your repeat list it may take longer. You can monitor the progress of your request via your Patient Access homepage.

Inappropriate Use

We monitor the use of this service and we are sure that you will find it useful. However, if we find any abuse of this service, we will revoke your access to the service.

Your Responsibility

The practice will take every measure to ensure that your Patient Access application is secure. It is your responsibility to ensure that your Patient Access account remains this way.
You are able to terminate or reset your account at any time by contacting the practice in writing. You may wish to do this if you think someone else knows your login details or if you have shared details with a family member or partner and no longer wish for them to know the details.

The University Medical Group reserves the rights to remove Patient Access from any patient without notice. This will in no way affect your registration at this practice.

Please note that the practice does not manage the Patient Access website and therefore cannot provide help with using the website. Help screens are provided on the website.

Proxy Access to Online GP Services

If you would like to have the facility to book appointments, request prescriptions etc for a dependant, for example a child or elderly parent we are now able to arrange a patient access by proxy account.

In order for us to arrange this, it is necessary for the person giving permission and the representative to complete the form, signing to give consent. If the patient does not have capacity to consent to proxy access and it is considered to be in the best interests of the patient Section 1 of the form may be omitted.

Download the Patient Access Proxy registration form which you will need to complete and return to reception along with TWO pieces of ID , for the representative(s), one piece of photographic identification and one recent non photographic correspondence (e.g. bank statement etc, please note, we are unable to accept utility bills) to complete the process.
