Primary Care Networks

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs). To find out more, click here and here. So far, being part of the PCN has allowed us to to have an in-house pharmacist, social prescriber, health coach and physiotherapist!

Sickness Certificates

The first 28 days of sickness do not require a certificate from your GP. If your employer insists on a sickness form, complete a Self-Certification form, which can be found here

Please ask staff to issue the usual sick notes, NOT the GP's.


We do NOT do sick notes for suspected Coronavirus

Sick Notes do NOT need to be SIGNED. We will text them to you, as we do not want you to come to surgery to pick them up. You can print them or email them to your employer.

We are not allowed to write letters telling your employer that you should be allowed to work from home. That's your HR department's decision, based on what medical conditions you declare to them. If they don't believe you, we can write a letter confirming your medical problems for £30 in advance. We have been overwhelmed with such requests, understandably, and we cannot guarantee a time frame for this.

Please do update your contact details ("Your Contact Details" box at the bottom of screen) with home & mobile & email. You will receive appointment reminders and occasional requests for information by text.

If you want to be able to; 

  • order repeat prescriptions
  • see parts of your health record, including information about medicines, vaccinations and test results
  • book, check or cancel appointments with a GP, nurse or other healthcare professional

Doctorlink

The Chesser Surgery would like to invite you to join DoctorLink.

We have been looking at how we can improve access to Practice services; DoctorLink is our new online service where you can access advice and book appointments online.

Sign-up is simple and you can create an account here https://app.doctorlink.com/chesser/register

For all urgent appointments, we ask that you undertake a symptom assessment through DoctorLink.

The symptom assessment will ask you a number of questions to help to identify your problem.

The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self-care.

If your outcome indicates that an appointment is required at your GP Practice, you simply click on the “book an appointment” button and book your appointment directly.

If no appointments are available, you will see a “request an appointment” button, please click this.

Once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified time frame (as outlined in the outcome of your symptom checker). 

The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.
