Patients Not Seen Within 3 Years

Patients aged between 16 and 75 who request a consultation and who have not seen a clinician within a period of three years prior to the date of request will receive a health check as deemed appropriate by the clinician in the course of the consultation.

Patients Aged 75 and Over

Patients who request a consultation and have not been seen by a clinician in the preceding 12 months, will receive a health check as deemed appropriate by the clinician in the course of the consultation.

From April 2014, all patients aged 75 and over were allocated a named doctor who will have overall responsibility for the care and support that our surgery provides to them.  The doctors will also work with other relevant health and care professionals, who are involved with patients to ensure that the correct care package meets their individual needs.

Immunisations

We offer a full immunisation programme for Children. Full details of the programme are available from your Health Visitor.
Seasonal Flu Vaccines.

We offer the seasonal flu (also known as Influenza) vaccine and the pneumonia vaccine to our patients who fall into the ‘At Risk’ groups. These include patients with diabetes, asthma, heart disease, etc and patient aged over 65. You can check with reception staff if you are unsure whether you are entitled to a flu vaccine or not.

Tele-Dermatology Service

We offer a tele-dermatology service.

If you have any new/worrying/changing lesions/moles etc we can arrange for you to come into the surgery and have photographs taken. These are then sent electronically to the Dermatology Department at the Hospital and the consultants will review the photographs and advise the practice on the best course of action.

Sound Doctor

The Sound Doctor film library is available for all patients within Barnsley.  There are around 300 films which adhere to NICE Guidelines, have been clinically reviewed and contain some of the best national and international speakers on each subject.  They have been co-created with leading charities including Diabetes UK, British Lung Foundation and Alzheimer's Society.

There are excellent evaluations showing that people go to the GP less and have fewer avoidable hospital admissions. One of the reasons for this is that there are between 40 and 60 films for each condition and they form a proper patient journey than small snippets of information which are not significant enought to help the self-management process.

They are easy to use.  All patients have to do is email: info@thesounddoctor.org and say which their GP practices is.  They are then sent a link to click on so that they can then create a username and password and have access to the film library.

It is clearly laid out in conditions and sections and is very easy to navigate around the site too.  Conditions covered include:

*  Diabetes

*  Heart Failure

*  COPD

*  Dementia

*  Weight Management Surgery

*  Back Pain
