Please then book an appointment with a nurse and bring your completed form with you.
If you do not being your completed form with you we cannot address your travel needs safely in the allotted appointment time and may have to send you away to rebook.
At this initial appointment the nurse will discuss your requirements. You may not receive any vaccinations at this first appointment, but, if necessary, appointments will then be made for your vaccines.
If you are going on an extended, complex trip visiting several countries, please book a double appointment initially.
We recommend you book your appointment a minimum of 6 weeks before you are due to travel, as some vaccines are given as courses over weeks.
The surgery does not have last minutes appointments available, and any travel vaccination should be given a minimum of two weeks before you go, so please plan your trip to the surgery well in advance.
If you book in with a nurse too close to your travel date we may not be able to offer you vaccines. Alternatively, you could visit a travel clinic.

It is not possible to provide all services under the National Health Service and the following tables show the current level of private fees and the services for which you may have to pay.

NB: If you require a Yellow Fever Vaccination and are not a registered patient at the Glastonbury Surgery, you and your registered GP will need to complete this form . Please remember to allow sufficient time for this to be done and for you to collect it from your GP prior to your appointment with us.

The practice provides a confidential drop-in service for everyone aged 13 - 18. It is held every Thursday between 16:30 - 17:30 and young people who need treatment or advice about sexual health, contraception, eating disorders, drugs, counselling or general health matters are welcome to see either the doctor or practice nurses.
No appointment is necessary and patients are seen in turn as they arrive.
The Glastonbury Surgery is a Somerset C-Card Issue point. This is a totally free service offering condoms and sexual health information to all those aged 13-19.
You will need to have a quick chat with our trained C-Card issuer in order to sign up to this scheme, then you will be able to access condoms at any C-Card issue point across the country.

With No Scalpel Vasectomies, the surgeon makes one tiny puncture with an instrument, which is then used to gently stretch the opening it has made so that the vas deferens can be reached and blocked using the same method as conventional vasectomies. There are no sutures so the wound heals far more quickly leaving no scar.
Dr Strawford is an experienced vasectomy surgeon and has received extensive training in this procedure and is a member of the British Association of No Scalpel Vasectomists. He offers no-obligation consultations to discuss all aspects of the procedure which is free of charge with the NHS at Glastonbury Surgery.
You should be able to return to light work the day after the procedure. Although please allow 2-3 days to be on the safe side. If your job is very physical or involves heavy lifting, a week off work is recommended.
There is little discomfort with the local anaesthetic and once this has worn off any pain felt is only slight and can be treated with paracetamol or ibuprofen.
The actual procedure will only take 20 to 30 minutes.
The No Scalpel Vasectomy has very low rates of post operative complications of bruising, swelling, and pain; and is now the preferred method of sterilisation as endorsed by the Family Planning Association and Royal College of Surgeons.

Health Connections Mendip is a new health and wellbeing service available to patients.
Patients can talk to a health connector at the surgery to find out about local activities, support and services available, and can also arrange one-to-one appointments to speak to a Connector.
During these appointments a Health Connector will listen to your health story and assist you with setting health-related goals which are meaningful to you.
Health Connectors also support you to make positive changes that last.
