All patients aged 40-74 years of age are eligible to a free NHS Health Check every 5 years. This check will include lifestyle advice and screening for possible signs of heart disease, stroke or diabetes.
All patients diagnosed with a chronic disease such as diabetes, asthma, COPD, epilepsy, heart disease, CKD (Chronic Kidney Disease) or stroke are eligible for annual health checks and 6 monthly monitoring if medically required.
You are invited to have an annual check up with the nurse. This can be arranged at the surgery or at home if you are housebound.
Patients identified as being at risk of diabetes are monitored with the offer of an annual blood test. A referral for support and advice on lifestyle can be arranged.
All women aged 25-64 are eligible for cervical screening every 3-5 years. These checks will include a general health check and advice about lifestyle. Ladies who have had a hysterectomy and don't need smears anymore can still attend for a well women check.
The practice offers a child health surveillance service for all children under five years of age. As well as carrying out routine developmental checks.
The open access baby clinic for weighing your baby is run by our Nursery Nurse at the surgery every 2nd and 4th Friday of the month from 09:30 - 11:30.
Rhyme Time for children upto 6 months of age is every Tuesday from 10:00 - 11:30.
Our doctors provide joint injections and cryotherapy, any other minor surgery requirement will require a referral to another NHS site.
The nursing team run a leg ulcer clinic twice a week for patients who need regular dressings and care for this condition.
Chiropody clinics are held in the building for those patients who are entitled to free chiropody treatment - referral by a GP or nurse is required.
The practice has access to two registered mental health nurses who are specially trained to work with patients who may be experiencing mental health problems or dementia. Following a consultation with one of the GPs you may be referred to these nurses for further treatment, advice or support.
Due to excess demand for GP appointments, the practice employs a Practice Nurse who has advanced training in Minor Illness. The nurse can diagnose, treat and arrange prescriptions for many minor illnesses including:

Skin rashes Skin infections Urine infections Women's health problems
If you think your problem could be treated by the Practice Nurse ask the receptionist or speak to the triage nurse.
Appointments can be booked for ear syringing, removal of sutures, dressing of wounds, injections etc.
If you have musculoskeletal problems and are aged 16 or over your can now refer yourself for a course of Musculoskeletal (MSK) Physiotherapy. Childhood immunisations are available by appointment with our four trained practice nurses. If you wish to discuss the vaccinations before going ahead then the doctors or practice nurses are happy to answer and queries you have.
HPV vaccination against cervical cancer will be offered to girls at the age 12 by the school nurse and the tetanus, diptheria and polio booster and meningitis ACWY will be offered to all children at 14-15 years of age at school. The practice nurses are happy to vaccinate any children who have missed their booster at school.

* These vaccinations will be routinely offered at school but can be given by the practice nurses if you child could not attend the school vaccination programme.
All the doctors provide confidential contraceptive and emergency contraceptive advice to patients in all age groups. We have trained doctors and nurses who can fit coils (IUCDs) and contraceptive implants. Gentlemen who wish a vasectomy can be referred to local services.
We have a family planning trained nurses who offer a comprehensive service and advice to both sexes. Nurses offer booked appointments during the day and in an evening.

Guidelines recommend all patients on the contraceptive pill attend an annual review to simplify this for patients we are setting up a new system for ordering contaceptive pill prescriptions.

You nolonger need to book an appointment with the nurse. Simply complete the form below (copies are available at reception if you do not have access to a printer). Either book an appointment with the HCA for your blood pressure check or use the pod in the waiting room.
Please note your contraceptive prescription will not be available until 48 hours after your BP check.
Contraceptive Pill Check List
If you are planning a pregnancy or have just tested positive, the nurses are available by appointment to provide you with advice about Folic Acid, smoking cessation, diet, exercise and alcohol during pregnancy.
After 20 weeks of pregnancy all ladies will be offered a Whooping Cough vaccination to help protect your baby from this infectious disease.
During September to March all pregnant ladies will be offered a vaccination against influenza.
If you require any vaccinations relating to foreign travel you need to complete the travel form below and we will contact you to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to complete this form as early as possible - at least 6 weeks before you travel - as an appointment may be required with the practice nurse to actually receive the vaccinations. These vaccines may have to be ordered as they are not a stock vaccine. Your appointment should be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private basis and these incur a charge for the cost of the vaccine. This is because not all travel vaccinations are included in the services provided by the NHS.

The fees charged are based on the the cost of providing this service and our reception staff will be happy to advise you about them along with appointment availability.
Any samples requested by the doctor or a nurse should be placed in the receptacle provided at reception please put your sample container in a plastic bag or envelope for hygiene reasons.
Please ensure the sample is labelled with your name, date of birth and GP. All samples brought to the surgery for testing should be received by 16:00 unless specifically instructed by a nurse or GP.
If you require test results and the doctor has advised you to telephone for these, it would assist the receptionists if you could ring 254600 between the hours of 10:00 and 11:30 or 14:00 and 17:00 when the phones are less busy.
