Online access 

Your online access registration details are now available from our reception teams at St Nicholas', St Patrick's and Hodgson Lane Surgeries. Please ensure you bring in your identification i.e passport or driving license to enable us to release this information. 

Late Night Appointments

We are pleased to announce that we now have late night opening on Tuesdays and Wednesdays to 8pm for nurse and Healthcare Assistant appointments.  We hope this will prove beneficial to patients who struggle to make a day time nurse appointments.  You can see our Healthcare Assistants Danielle & Jessica for the following: BP's, Healthchecks, Dementia Screening, Vascular checks, Stroke/TIA checks, Hearing Tests, Mental Health Reviews, Dopplers, B12 injections, ECG's. 


A small number of online appointments are available to book. This includes online booking for telephone appointments.

We continue to have a very high number of patients who fail to attend appointments they have made each day so please do remember to cancel so they can be offered to other patients who may really need them . You can cancel your appointments online. 


chdThe practice has developed a new innovative approach for managing patients with Long Term Conditions. If you have asthma, COPD, diabetes, ischaemic heart disease, epilepsy, high blood pressure, heart failure or have had a stroke/TIA you might receive an invitation to this new clinic. The project is currently targeted at those patients who have more than 1 of the above problems but hopefully will be rolled out to all patients with Long Term Conditions in future years.


The clinic consists of one single longer appointment for all your conditions. Prior to the appointment you will be sent a Health Needs Assessment form to complete so we can concentrate the appointment on the areas which concern you the most. A GP & the practice pharmacist will also review your record; before your appointment; and might make suggestions to the practice nurse to improve your care.


The appointment will focus on increased education, creating personal goals & encourage self care/management. As part of the appointment you will create an agreed personal care plan for the next 12 month period.
