Introduction - What is Enhanced Access?

From 1st October 2022, GP practices will be providing increased provision of primary care services (enhanced access arrangements) between the hours of 6.30pm and 8.00pm Mondays to Fridays and between 9.00am and 5.00pm on Saturdays. Our building will be hosing this service. If you prefer to be seen in the evenings or weekends please contact the surgery as normal and book an appointment. 

We have a number of different clinicians working with us, GPs, GP Trainees, Practice Nurse, Nurse Associate, Health Improvement Practitioner, Pharmacists, Mental Health Practitioner, TAPP, Paramedics & MSK Practitioner - to help us book you in to see the correct person - Reception will be asking you for your name/dob/tele no & what your appointment is regarding, this is to ensure that 1) you are one of our patients and 2) that you are booked with the correct person which will give them time to prepare for your appointment. You also have the option of either a face to face or telephone appointment.

 

We are taking on NEW patients, if you are in our practice are and would like to register, please pop into the surgery for more details, we are on the right as you come through the door!

 

Pharmacists

Jane Melia (f)

Jane is our Pharmacy Technician. She works across all 6 practices in our Primary Care Network. She joined us in 2020. She supports the practice with our drug safety audits which we do to ensure that the practice are prescribing safely and effectively. She works alongside the pharmacists and assists with medication queries, reconcilling medication after a patient has been in hospital, requests from the hospitals to change any of their current medications.

Amirah Khan (f)

Amirah is currently on maternity leave. Amirah is one of our Pharmacists and with us all day Tuesday and holds one clinic per week in which she sees patients who need a 6-12 monthly review of their medications. She then supports the doctors with medication queries, reconciling medication after a patient has been in hospital, requests from the hospitals to change any of their current medications.

Raeesa Ali (f)

 Raeesa is one of our Pharmacists and is with us Monday afternoon and holds one clinic per week in which she sees patients who need a 6-12 monthly review of their medications. She then supports the doctors with medication queries, reconciling medication after a patient has been in hospital, requests from the hospitals to change any of their current medications.

Nabeelah Illahi (f)

Nabeelah is one of our Pharmacists and is with us Thursday morning and holds one clinic per week in which she sees patients who need a 6-12 monthly review of their medications. She then supports the doctors with medication queries, reconciling medication after a patient has been in hospital, requests from the hospitals to change any of their current medications.

Sahra is our new Pharmacy Technician. She works across all 6 practices in our Primary Care Network. Stephen Fielding (m)

Steve is one of our Paramedics and he joined the practice in April 2021.

Steve works with all 7 practices of our Primary Care Network.  He holds 2 clinics per week at the practice.

Steve supports the doctors, seeing patients and visiting. He is able to assess your problem, examine you, organise any tests that may be required and decide, with you, a course of treatment.

David Haslam (m)

David is one of our Paramedics and he joined the practice in July 2021.

David works with all 7 practices of our Primary Care Network.  He holds 2 clinics per week at the practice.

David supports the doctors, seeing patients and visiting. He is able to assess your problem, examine you, organise any tests that may be required and decide, with you, a course of treatment.

James Limbert (m)

James is our Musculoskeletal (MSK) Practitioner and he joins the team on February 2021.


He is the first point of contact for patients who may have the following:


**All soft tissue injuries, sprains, strains or sports injuries – upper and lower limb


** Arthritis – any joint


**Possible problems with muscles, ligaments, tendons or bone e.g. carpal tunnel syndrome, tennis elbow, tendon ruptures


**Spinal pain including neck and lower back pain


**Spinal pain including arm/leg symptoms – may also have neurological symptoms  e.g. pins and needles, numbness


**Post orthopaedic surgery


**Mobility changes


He will be able to:


*Assess MSK problems


*Give advice on self-management


*Provide a simple exercise programme


*Refer to Physiotherapy if require further input


If you have MSK condition - i.e. low back pain, hip pain, knee pain, arm pain etc. you will be able to book an appointment with our MSK practitioner instead of seeing the doctor.

Cameron Fleming (m)

Cameron, Musculoskeletal (MSK) Practitioner and he joins the team on Monday 19 June 2023.

Jennifer Thompson (f)

Our Mental Health Practitioner is Jennifer who works Monday and Tuesday mornings.

She is an experienced qualified mental health specialist who is employed by the Greater Manchester Mental Health Foundation Trust and commissioned by the Bolton Clinical Commissioning Group.  He will be working as part of a neighbourhood of practices and will be running clinics for our patients at our surgery.

*First point of contact at the practice if you have anxiety, depression, low mood.  You do not need to see the doctor first, Ian will have the better expertise to manage your symptoms.

*Second point of contact if you have seen a doctor and you or the doctor feels you require further advice or treatment.

She can provide brief interventions, spend more time exploring your symptoms/problems and provide specialist advice/ongoing referrals to the Primary Care Psychological Therapy Service (where required)/safety and management plans.

They will also provide support to our team of doctors and nurses.

Please be aware that you will be asked what you are booking an appointment in when you ring the practice to arrange an appointment so that our Reception Team can book you in with the best person to manage your symptoms/problems.  We appreciate you co-operation so we can help you.

Andrea Hollman (f)

Andrea is our social prescriber link worker.


It’s estimated that one in five of the people who go to see their doctor are troubled by things that can’t be cured by medical treatment, like the effects of poor housing, debt, stress and loneliness. Many people are overwhelmed and can’t reach out to make the connections that could make a difference to their situation. This is especially true for people who have long-term conditions, who need support with their mental health, who are lonely or isolated, or who have complex social needs which affect their wellbeing.


And that’s where our social prescriber link worker comes in.  Andrea can help people to identify what matters to them, and work out how to connect with the activities that might make a difference, giving people a sense of belonging that comes from being part of a community group. This can help people to stay physically and mentally well for longer and manage the long-term conditions they might be living with.

Megan is our Care Co-ordinator

Marie-Louise Hatungimana (f)

Marie-Louise is our Health Improvement Practitioner at the practice.

She works alongside our clinical team to support health prevention programmes.

Her clinics are for the following patients:

*Patients at risk of developing diabetes.

*Patients at risk of developing a cardiovascular disease.

*Patients at risk of alcohol harm

*Patients aged 40-75 with no precluding cardiovascular disease - NHS Health Check

*Cholesterol Management

*Stop smoking support

*Weight Management

***Coming soon - supporting patients with newly diagnosed diabetes or patients with mild to moderate COPD - support with lifestyle advice to help manage their chronic diseases better***
