A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing. First Contact Practitioner (FCP) Alan Goldson-Truman As a First Contact Physiotherapist (FCP) my role is to assess patients musculoskeletal conditions ranging from painful knees, backs, shoulders and any other painful joint or muscles. After the assessment we establish a treatment plan for you to complete at home, or if needed, arrange for onwards referral to other specialists In order to further investigate your condition. Mary Grace, Care Co-ordinator Mary can identify your care and support needs and will work with you to connect you to the right services or professionals at the right time. Care Co-ordinators act as a central point of contact to ensure support is available to help patients to manage their care. Simarone Sahota, Social Prescriber Simarone can connect you to community groups and non-clinical services for practical and emotional support. If you need support with lifestyle, debt, employment, managing stress, anxiety, bereavement, loneliness etc, or if you would like to promote general well-being, our social prescriber can refer you into local exercise groups. Our Social Prescriber can link you to support within your community to help you work towards a more fulfilling life. |
