Primary Care - Mental Health Worker If you are feelin sad, anxious or experiencing low mood then telephone or face to face appointments are available with our Mental Health Worker. Please just speak to a member of our team to make an appointment. Primary care mental health workers support GP surgeries as first points of contact for patients experiencing mild to moderate mental health illnesses, reducing the wait or even need, to see a GP. Jane is an experienced mental health worker and works with our GP's, supporting the practice to assess people who contact them with mental health related illnesses, ensuring they receive proactive, effective and timely care as well as additional signposting advice and information if needed. Jane will also liaise with relevant services where necessary, to ensure patients receive the right referral first time and as quickly as possible. She will also liaise with your GP and arrange for you to start mediation where necessary. |

Social prescribing link workers give people time, focusing on 'what matters to me?' Social prescribing particularly works well for people who: - have one or more long term conditions
- who need support with low level mental health issues
- who are lonely or isolated
- who have complex social needs which affect their wellbeing
