Covid 19 Vaccinations

We are currently sending out text messages inviting people to book an appointment for a Covid 19 vaccination at Sharley Park Leisure Centre.  If you receive a text message, please follow the link to book your appointment.  If you already have an appointment or have had your vaccine please ignore the message.  If you have not yet received an invitation please wait until you are called as there are a limited amount of vaccinations each week and we may not have reached your age range. Please DO NOT TELEPHONE THE SURGERY as we are extremely busy and will advise you as above.

We have made a slight change to our prescription form. The form is very similar to the existing form and self explanatory but you will need to delete any bookmarks you have to the existing form as it will no longer be available.


Repeat Prescriptions

You can now order your repeat prescription online via the link at the top of this page.

To order a repeat prescription please tick the items you require on your print out from your previous prescription.

You can either send your repeat prescription request to the surgery by post or by hand delivery to the post box next to reception or through the letter box if the surgery is closed. If you have a query regarding your medication you may hand your repeat prescription to a receptionist over the counter.

Collections

Repeat prescriptions received after 08:00 at the surgery will be available for collection after 13:30 the following working day. If there are any problems with requested medication then this may take longer.

If you prefer you may attach a stamped addressed envelope to your request and we will post it back to you.

Some of the local pharmacies offer a repeat prescription service; please ask at your pharmacy if you require more information.

Please note we are unable to deal with your repeat prescription request over the telephone or via fax.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
