With this website we aim to help you find all the information you need at the click of a mouse. You can order repeat prescriptions online, cancel existing appointments, change the contact details we hold for you and much more. 

NEW!  Update Your Records
You can now update some basic information in your clinical record - just click on the Your Clinical Record link at the bottom of this page and an online form will open which you can use to send us details about your height, weight, waist measurement, smoking status, if you are a carer or if you have any allergies.  You can also inform us of your ethnicity.  This information will then be added to your computer record at the surgery by a member of staff.

Ordering Your Repeat Prescription Online
Just click on the Prescriptions tab on the left hand side of this text or the Repeat Prescriptions link at the bottom of this page, complete the online form and click Send Request. You will receive an e-mail confirming that we have received your request. Prescriptions will be available for collection within 48 hours (excluding weekends and Bank Holidays) from receipt of your order.

Half Day Closures
We close for one afternoon (12.30pm - 5.00pm) a month for training purposes. The dates for the next three months are as follows:

Tues 5th August
Wed 3rd September
Thurs 2nd October

If you require advice or an urgent home visit when we are closed on these afternoons please telephone (01283) 563631. The surgery re-opens at 5.00pm.

Surgery Car Park
If you travel by car to the surgery, please ensure that you drive slowly and carefully keeping to the left through Holly Green. Please park ONLY in the surgery car park or preferably use nearby public parking areas. Holly Green is private and its parking areas are for permit holders only. Thank you for your co-operation.
