For many of our patients we don’t have up-to-date details of Smoking and Alcohol consumption. It would be of great assistance to us if you could click on the link at the bottom of the page “Your Clinical Record” and fill in these details. Please be assured that none of your personal details will be used outside this practice.

It would also be appreciated if you could update your contact details; telephone numbers and email address.

By keeping your details up-to-date you help us to give you the most appropriate care. You’d be surprised how many of our patients don’t keep us informed of changes!

When the Surgery is Closed
The service provided out of normal practice hours is for emergencies, which cannot wait until your surgery re-opens.  Please contact the practice in the first instance and your call will be transferred to the “out of hour’s” service.

Windhill Green & Cliffe Avenue surgeries serve the communities of Baildon, Shipley and Thackley . Our doctors, nurses and all our other staff are dedicated to offering a professional service and this surgery website will help us to keep all our patients up to date with news and information about our practice. We will keep it current with any developments or other details that we feel are relevant to you.
