Online access 

Your online access registration details are now available from our reception teams at St Nicholas', St Patrick's and Hodgson Lane Surgeries. Please ensure you bring in your identification i.e passport or driving license to enable us to release this information. 

Late Night Appointments

We are pleased to announce that we now have late night opening on Tuesdays and Wednesdays to 8pm for nurse and Healthcare Assistant appointments.  We hope this will prove beneficial to patients who struggle to make a day time nurse appointments.  You can see our Healthcare Assistants Danielle & Jessica for the following: BP's, Healthchecks, Dementia Screening, Vascular checks, Stroke/TIA checks, Hearing Tests, Mental Health Reviews, Dopplers, B12 injections, ECG's. 

Appointments

A small number of online appointments are available to book. This includes online booking for telephone appointments.

We continue to have a very high number of patients who fail to attend appointments they have made each day so please do remember to cancel so they can be offered to other patients who may really need them . You can cancel your appointments online.

Right to Access

You have the right to obtain:

  • confirmation that your information is being used, stored or shared by the practice
  • a copy of information held about you.

We will respond to your request within 30 days of receipt or will tell you when it might take longer.

We are required to validate your identity including the identity of someone making a request on your behalf

In the first instance you will granted access to your online records.  To obtain access to these records you must present in person at the surgery with photographic ID. You will then be given a login and password and you will use these to access via:
