The Park View Patient Participation Group was formed in 2012 to encourage an ongoing culture of feedback and patient involvement. The group is not a forum for an individual issue or complaint, but a voice of the patient contributing to the modelling of healthcare and services accessed by all, and is made up of our patients and representatives from the Practice.

Patient Forum e-mail contact form
By providing your email details we can add them to a contact list so we can email your occasionally to ask you a question or two. We will only ask general questions about the practice, such as short questionnaires. We will not ask medical or personal questions. Your responses will help us assess how well we are doing to identify areas for improvement and ensure that any changes that are being made are patient focused. Your contact details will be kept safely and securely and will only be used for this purpose and will not be shared with anyone else. If you no longer wish to be in the virtual patient group contact list or if you leave the practice please advise us by e-mail and we will remove you from the contact list.
You can make a difference and gain an understanding of the complex world of the NHS, the difficulties we have to work through and perhaps play a part in developing services closer to home in the community.
The group is made up of our patients and representatives from the Practice. Sue Evans (Practice Manager) chairs the meetings and various members of our team including GPs may attend. Sue Cox (Patient) is our Secretary.
We aim to gather Patients from as broad a spectrum as possible. To get a truly representative cross section of our population we need young people, workers, retirees, people with long term conditions, people from non-British ethnic groups.
