To help us with this, we are setting up a Patient Representative Group(PRG)

Are YOU interested in finding out more about the Practice? and would YOU like to influence the development of local health services?

If so we want to hear from you!

The Practice has set up a Patient Representative Group (PRG) for our patients to discuss health services and local healthcare initiatives.

If you would like to join the Group, or require more information, then please pick up a leaflet from Reception, or download a copy here.

Completed forms can be hand delivered to the surgery or posted to:

We aim to gather around a dozen patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
