Register online - please complete all 3 steps (16+ Only)

  1) Fill in the Online form in Cymraeg or English,

• We need your full term time address, including room and block number.
• Please ensure to provide your NHS number, this can be obtained from your previous GP practice.

  2) Also download Patient Documents and fill in the required pages (listed on page one)

  3) Email the patient documents and proof of address to contact.w92025@wales.nhs.uk.

• If you take regular medication, please include your repeat prescription form with your application to register. For more information on prescriptions please see here 

If you do not complete all 3 steps your registration will not be processed. We will contact you via the email address you provide if there is a problem with your registration.

 Alternatively you can collect a registration pack from reception - please collect a pack from reception if you wish to register your child.
