New Patient Registration
Our lists are currently open to new patients who live within or are visiting the practice area. Visitors to the practice area will be temporarily registered for up to 16 days.
If you wish to register as a patient with the surgery please complete the purple GMS1 form and additional registration documents required by the surgery, these can be found within the reception area of any of our three surgery sites (listed below) or from the links below. Please not there is a different form to fill out for adults and children. You will also be asked to provide proof that you are living within our practice area as well as photographic ID, because of this we are unable to accept registration requests via post or email.

You may fill out these forms and bring them to any of the three surgery sites and you may be asked to attend your appointments at any of the three sites. The addresses of the three sites are as follows

The NHS App
The NHS App provides a simple and secure way for people to access a range of NHS services on their smartphone or tablet.
You can use the NHS App to get health advice, book appointments, order repeat prescriptions, view your GP health record including blood test results, and more.
If you already use EMIS online you can continue to use it. You can use the NHS App as well.

Friends and family test
The Friends and Family Test (FFT) is an important feedback tool that supports the fundamental principle that people who use NHS services should have the opportunity to provide feedback on their experience. Listening to the views of patients and staff helps identify what is working well, what can be improved and how.
The surgery have made paper copies of the FFT available at their reception desks, you can also access the survey by clicking here.
