The District Nurses attend patients who need nursing assistance at home; they will assess and discuss your individual needs with you and your carers during an initial visit, and arrange subsequent nursing care as required. Community Matrons are senior nurses who work as part of the rehabilitation health and social care team, managing patients with long term conditions e.g. Diabetes, Heart Disease and COPD. Please ask your GP to refer you to a Community Matron if you think you may benefit from this service. |
