Register as a new patient

To register with the practice you will need to either come in to the surgery and complete the registration forms and if possible, show proof of identity, or you can complete the registration form on your computer at home, print it out and bring it in to us with your proof of identity.

When applying to become a patient there is no regulatory requirement for you to prove your identity, address, immigration status or the provision of an NHS number.  However, there are practical reasons why we ask for proof of ID, so that we can be assured that people are who they say they are or to check where you live, so if you can provide the ID documentation, this will help with the process.  If the Practice suspects a patient of fraud, for example using fake ID, we will hand the matter over to the local NHS counter-fraud specialists.

When you register we can accept the following as proof of ID.

Please choose ONE from EACH of the following lists:

LIST A (Proof of ID)

Driving Licence
National Ins Card
Birth Certificate
LIST B (Proof of Address)

Paid utility bill/Official change of address
Local Authority rent card
Bank or credit card statement
Wage slip

Please be aware of the following BEFORE you complete your registration:

Patients register with the Practice, not with an individual GP. When making appointments you may express a preference as to which doctor you would like to see. However, if you request a particular Doctor please be aware there may be a wait to see him/her.

Patients aged 16 years old or older are medically considered adults. If you are registering a 16 year old then they will need to complete an adult registration form.

It is important for us to be able to contact you, without consent from you we will not be able to call / text / email you with information.

Registration Forms

You will need to complete the GMS1 form for all adults and all children registering at the Practice.
The GMS1 form is a PDF and will need to be printed out and completed by hand, or you can collect one from the surgery.

You can complete the Adult and Child Registration Forms electronically on your computer at home or work. Once you have completed all sections, please save to your computer and print out a copy. Please bring that copy, along with your GMS1 and your proof of identity, to either Cuckfield or The Vale surgery.
