It is increasingly possible for patients to self-refer directly into other services to receive treatment. This helps to reduce the demand for GP appointments and is more convenient for the patient.

Physiotherapy

This Out-patient Physiotherapy Service provides assessment, diagnosis and treatment for a wide variety of musculoskeletal problems (i.e. injury, damage or disorders of the joints and other tissues e.g. bones, ligaments, tendons, muscles, nerves). It also offers advice and education on the self-management and prevention of musculoskeletal problems.

Physiotherapy

This Out-patient Physiotherapy Service provides assessment, diagnosis & treatment for:

  • Pelvic floor problems (bladder or bowel urgency or leakage).

  • Pelvic or genital pain

  • Pelvic organ prolapse

  • Prenatal or postnatal muscle, nerve or joint pain

Neurological Physiotherapy

This Out-patient Physiotherapy Service provides advice, education, exercise, equipment or onward referral for patients who have a neurological condition diagnosed by a neurological consultant. Conditions include:

  • Multiple Sclerosis (MS)

  • Parkinson's

  • Previous stroke

  • Cerebral Palsy

  • Peripheral Neuropathy

Mental Health & Talking Therapies

 

This therapy service is provided for patients over 16 years of age with the following problems:

Podiatry (foot care)

The Community Podiatry Service provides assessment, diagnosis and treatment to a range of patients with lower limb and foot problems, including:

  • People with long term conditions such as diabetes, vascular disease, amputees, connective tissue disorders, stroke, Parkinson's disease

  • People with multiple and complex needs e.g. dementia, falls

  • People with biomechanical problems e.g. gait /postural problems

  • People requiring nail surgery

  • People with severe foot and ankle problems and conditions

Drug & Alcohol

Derbyshire Recovery Partnership provides services for people in Derbyshire with a drug and/or alcohol problems. The teams offer different levels of support from brief advice and harm reduction to intensive structured 1-to-1 and group work.

Derbyshire Recovery Partnership also manages all substance misuse substitute prescribing for drug or alcohol treatment across Derbyshire.

The service is for any adult (18+) who wishes to address any issues they have caused by the use of drugs or alcohol. This can include binge drinking, the use of New Psychoactive Substances, as well as physical alcohol or drug dependence.

Derbyshire Recovery Partnership (DRP) is a drug and alcohol treatment service between Derbyshire Healthcare NHS Foundation Trust, Derbyshire Alcohol Advice Service (DAAS), Phoenix Futures and Intuitive Thinking Skills.

Weight Management

This service is for anyone over 16 years old & with a Body Mass Index (BMI) over 25. Calculate your BMI here.

The service supports you to set and achieve your own weight loss targets and to maintain this weight loss long term. It is different to other weight loss programmes as it focuses on what’s happening in both the body and the mind to help you achieve long-term weight loss and wellbeing.
