Access to Online Services

We are now able to offer online access to your coded medical record. Access is via the Practice's clinical system. To apply, please complete the registration form and return to the Practice Manager.

The request may take up to 14 working days to process. In some circumstances, it may take longer.

The Practice will not approve access if it is deemed that it may cause physical or mental harm to the patient.

Certification is required with two forms of identification, one of which must be photographic i.e., passport, bus pass. This is to ensure the Practice adheres to security guidelines.
