We would like to welcome requests for registration from patients residing or moving into the practice area.

You can register by bringing along your NHS Medical Card or completing a GMS1 form which is available from reception or can be downloaded from the above "Registering Online" tab.  Please note if you register using the online services you must attend surgery to sign the form before your application to register can be processed.  Alternatively you can print the form at home and return the signed form to the surgery.

In order to process your registration, patients must attend for a New Patient Check with the Health Care Assistant. Please ask at reception for details.

Please note that you medical records will be requested from your previous GP once you have completed the registration process but this may take up to 8 weeks. Please ensure that you have an up to date list of your medication when you attend.

Medical treatment may under certain situations be available from the date of registration. Please contact reception for further information.
